A broken appointment is a loss to everyone. Please be advised a charge of $75 will be added to your account if you don’t show up to your appointment or is over 7 minutes late without prior communication.

Please take time to complete the online patient registration prior to your next visit to VanVision Eyecare Center. If you are unable to fill out the registration form prior to your scheduled exam, please arrive at our clinic at least 30 minutes early to fill out paperwork. If your paperwork is not completed before your exam time you may be asked to reschedule.
